Request: shape function of plate

I need a shape function of plate in FEA, which need:
(1) For 4-nodes Quadrilateral element(that is, 12-DOFs exists in this element);
(2) Satisfies C0 coninuity condition;
(3) Satisfies second-order accuracy.

I'll appreciate your help. Please provide such shape functions as more as possible.
