physical/technical circumstances that led to the accident?

The spoil tips produced by the mining process were placed on top
of a layer of porous sandstone that housed a number of underground springs. On
the lead up to the disaster it had been raining heavily for days which caused approximately
3-6 meters of the spoil tip to subside.

Installing
solar panels on rooftops are obviously a high risk activity. There are a number
of hazards such as working at heights which could lead to falls from heights or
falling objects. Also slippery roof surfaces, damaged roofs, electrical
discharge from uninstalled panels, electrical fitting and equipments are all
potential hazards. All these, when not properly controlled could lead to
undesired conquences.

General area :

The battery is known for its strict requirement of the lowest maximum allowed working temperature in the entire electric transmission chain assembly.  This poses a problem particularly in climates with extreme temperature variations (35-45°C in summer and 0 to -30°C during winter) and during fast recharging at a significant battery power level.
