BOP pressure test incident

Hello ,

I would like to share an incident that happened recently in our rig. 

Facts Pressure Testing of
Shaffer BOP to 15000 psi ongoing. When pressure reached 4700 psi the lower
single ram door was blown off the BOP. Causes It was found that the door
locking mechanism was not engaged, due to human error. Corrective
measures/learnings

• Improve safety culture on
board

• Improve procedures for
maintenance and operation of BOP

• Crew training including OJTs
